Audit trail

An audit trail is the chronological record of who did what, when and through which system, kept so an action can be reconstructed and checked later.

Every approval, edit, payment and AI-generated response should leave a trace: who or what triggered it, when it happened and what changed. That record is what lets a company answer a dispute, pass a compliance review or work out what went wrong after an incident, instead of relying on memory.

AI adds a new wrinkle. An audit trail for an AI agent needs to capture not just that an action happened, but what the agent was asked to do, what data it used and why it chose that action, since a decision made by a model is harder to reconstruct after the fact than one made by a person following a written procedure.
